Your NPI has been verified. One signature completes your account, and it covers both patient samples and professional ordering. Once your signature is complete, please download a copy for your records.
What you are agreeing to: Samples are given free to your own patients and are never resold. Product bought at professional pricing is sold to your own patients, in your practice, in accordance with our suggested retail prices. The sale, distribution, or offering of Eosera products is strictly prohibited on any third-party ecommerce site (e.g. Amazon, Walmart.com, eBay, buy.com, Alibaba).
